The following forms are required for all swimmers and clubs. The Hold Harmless Agreement and Concussion information forms below should be included in your registration documents. The Midlakes Club Compliance Form needs to be completed and sent to the League along with the Insurance/Champ Entry fees check ($9 x number of swimmers per team) and the most up-to-date team rosters. For the 2010 season, please mail this information by June 1st to:

Midlakes Hold Harmless Agreement
Every Midlakes Swim club is required to have this form signed by each participating swimmer for the current season. Each club is required to have this information on file for the duration of the season.

This document can be used if you chose to modify it to include club information. You must, however, forward the altered document to the League for approval before distributing it to your club members.
